Forms

All volunteers must complete a Volunteer Application regardless of ministry or service. Minors (children under 18 years of age) must complete the Volunteer Application for Minors.

Volunteers working with children and/or vulnerable individuals must complete a full Volunteer Packet and the forms can be downloaded from the box on the right. Please return all eight sheets to the Business Office and be sure that each form is filled out completely. Please include a copy of your driver's license and the Driver Information Form if the ministry or service requires that you provide transportation to others. All screenings are valid for five years and allow the applicant to volunteer at any parish or school within the Archdiocese of Atlanta. All information is kept confidential.
