To add sick persons to the prayer list, we need:
His/her name, Parish Member?, Condition of person (Hospice/Hospital/Home), Description of Illness, Name and phone of person requesting Note: The names on this list are voided and we begin with a new list at the beginning of every month. If the sick person is to remain on list for the following month, a call needs to be made to the parish office at the beginning of every month.
To add deceased persons to the deceased list we need:
Name of deceased, Parish Member? (If not, please give relationship to Parish (i.e. mother of Parishioner), Name and phone of person requesting.
